He attended the University of Pennsylvania, receiving his A.B. in 1846 and A.M. in 1849, then received his M.D. from Jefferson Medical College in 1850. Bache served his residency at Pennsylvania Hospital (1852-1853).
He was attending physician, manager, and treasurer at the Children's Hospital of Philadelphia and director and vice-president at the Pennsylvania Institution for the Deaf and Dumb. Bache was elected to Fellowship in the College of Physicians of Philadelphia in 1852. He was also a member of the Philadelphia County Medical Society, the Medical Society of the State of Pennsylvania, and the Pathological Society of Philadelphia.
Founded the Children's Hospital of Philadelphia with Doctors Francis Lewis and R.A.F. Penrose.
First served as Chief Surgeon of the 17th Pennsylvania Volunteer Infantry from April 25, 1861 to August 2, 1861. He was then commissioned as Major and Surgeon, US Volunteers on September 4, 1861. He went on to serve in Cape Hatteras, Baton Rouge, New Orleans, Vicksburg, and then home to Philadelphia. He was brevetted Lieutenant Colonel, US Volunteers on August 26, 1865 for "faithful and meritorious services during the war".
He was the curator of the Mutter Museum from 1866 to 1885.
