Bache was elected to Fellowship in the College of Physicians of Philadelphia in 1852. He was also a member of the Philadelphia County Medical Society, the Medical Society of the State of Pennsylvania, and the Pathological Society of Philadelphia.

He went on to serve in Cape Hatteras, Baton Rouge, New Orleans, Vicksburg, and then home to Philadelphia. He was brevetted Lieutenant Colonel, US Volunteers on August 26, 1865 for "faithful and meritorious services during the war".
He was the curator of the Mutter Museum from 1866 to 1885.
