Aged 49, Pope John Paul II, appointed him as Auxiliary Bishop of the Metropolitan See of São Sebastião do Rio de Janeiro, receiving his episcopal consecration with the Titular See of Drusiliana from Cardinal Eugênio de Araújo Sales, assisted by Bishops Adriano Mandarino Hypólito OFM., and Waldyr Calheiros de Novais. Between 1991 and 1999, Dom Affonso served as President of the Cáritas Internacional, the umbrella organization for 162 National Catholic Charities, and later as president of the CERIS. He guided the Caritas Confederation as it responded to the end of the Cold War, the Genocide in Rwanda, and conflict in the former Yugoslavia. A bridge built over the Tocantins River has since been named after him.
